Job Description / Duties

The Community Navigator for the Lower-Limb Preservation Integrated Care Demonstration Program will work with HHS interdisciplinary teams, primary care, specialists and community service providers across the Greater Hamilton Health Network (GHHN) Ontario Health Team (OHT) to support the implementation and integration of the Hamilton Health Sciences and GHHN OHT Lower-Limb Preservation Integrated Care Demonstration Program, including; delivering diabetic foot wound care/management education to primary care and community health care providers, providing operational oversight to support program integration and flow across the lower Hamilton region, liaising with community partners to facilitate adoption and utilization of virtual wound care management, and collection of required data to support the evaluation of outcomes of integrated care delivery.

Qualifications / Required Skills

1. A Regulated Health Professional in good standing with their respective college.
2. Knowledge and demonstrated application of skin and wound best practice.
3. Recent 3 to 5 years of demonstrated experience delivering adult education, and strong presentation skills
4. Daily assistance to clinical teams around referral management, including connecting with community stakeholders to support access and flow.
5. Knowledge of local, regional, provincial and national health care systems and the interaction of these systems within the program.
6. Excellent communication, time management and organizational skills required.
7. Excellent critical thinking, problem solving, decision making and conflict resolution skills.
8. Strong leadership, interpersonal, negotiating, consensus-building and teaching skills.
9. Demonstrated ability to respond to partners and situations with flexibility and adaptability.
10. Demonstrated ability to work independently and collaboratively with an interdisciplinary team, primary care, community services.
11. Demonstrated knowledge of Microsoft Office software applications.
12. Must be able and willing to travel to community partner locations within the Hamilton region.
